About Head to Health Wellington
Head to Health Wellington is a federally funded mental health service commissioned by Gippsland Primary Health Network (PHN). The service is designed to improve access to free, culturally safe, and evidence‑based mental health care for people living in the Wellington Shire and surrounding communities.
Head to Health services complement existing mental health providers and support people to access the right care, at the right time, close to home.
Our Services
Head to Health Wellington is not a crisis service.
In an emergency, please call 000.
The hub provides care through a multidisciplinary mental health team, including:
- Psychologists
- Mental health nurses
- Social workers
- Alcohol and other drug workers
- Counsellors
Services are free and funded by the Australian Government through Gippsland Primary Health Network.
Who We Support
We support people of all ages, genders, and cultural backgrounds, with a particular focus on individuals and communities experiencing increased vulnerability, including:
- Children, young people, adolescents, and older adults
(including those living in residential aged care) - Farmers and people affected by drought, bushfires, and natural disasters
- People experiencing financial stress, employment difficulties, or family and relationship challenges
- People from CALD communities
- LGBTQIA+ individuals
- People experiencing alcohol and other drug concerns
Referrals
You do not need a GP referral to access Head to Health Wellington.
You can self‑refer by:

Our team will discuss your needs and help connect you with the most appropriate support.
Outreach Services
Head to Health Wellington delivers the outreach support, to support individuals who may not otherwise be able to access mental health care.
Our outreach services include:
- Mental health support for residents living in residential aged care facilities
- Outreach support for older people living at home
- Co‑located outreach services in Maffra (Johnson Medical Centre) and Yarram
Group Sessions
Our clinicians can provide group-based mental health sessions for vulnerable populations, covering a range of wellbeing and mental health topics.
