The service is designed to improve access to free, culturally safe, and evidence\u2011based mental health care for people living in the Wellington Shire and surrounding communities.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Head to Health services complement existing mental health providers and support people to access the right care, at the right time, close to home.<\/p>\n\n\n\n<div class=\"wp-block-group has-base-color has-secondary-higher-background-color has-text-color has-background has-link-color wp-elements-0a30acd9d89a4082bced0d39e79642f3 has-global-padding is-layout-constrained wp-block-group-is-layout-constrained\" style=\"border-radius:10px\">\n<p class=\"has-text-align-left wp-block-paragraph\"><span style=\"font-weight:700\">The Hubs are not a crisis services, in an emergency please contact 000<\/span><\/p>\n<\/div>\n\n\n\n<h2 id=\"our-services\" class=\"wp-block-heading\">Our Services<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The hub provides care through a multidisciplinary mental health team, including:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Psychologists<\/li>\n\n\n\n<li>Mental health nurses<\/li>\n\n\n\n<li>Social workers<\/li>\n\n\n\n<li>Alcohol and other drug workers<\/li>\n\n\n\n<li>Counsellors<\/li>\n<\/ul>\n\n\n\n<div class=\"wp-block-buttons is-layout-flex wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button\"><a class=\"wp-block-button__link wp-element-button\" href=\"https:\/\/inglismedical.com.au\/headtohealth\/head-to-health-team\/\">Meet the team<\/a><\/div>\n<\/div>\n\n\n\n<p class=\"wp-block-paragraph\">Services are free and funded by the Australian Government through Gippsland Primary Health Network.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/inglismedical.com.au\/headtohealth\/head-to-health-team\/\">Meet the team<\/a><\/p>\n\n\n\n<h2 id=\"where-to-find-us\" class=\"wp-block-heading\">Where to find us?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Head to Health Wellington is location at 10 Inglis Street, Sale VIC, adjacent to Inglis Medical Centre<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" id=\"opening-hours\">Opening Hours: Monday to Friday: 8:30 am \u2013 5:30 pm<br>Extended hours appointments are available by appointment only.<\/p>\n\n\n\n<div class=\"wp-block-group has-neutral-higher-background-color has-background has-global-padding is-layout-constrained wp-block-group-is-layout-constrained\">\n<p class=\"wp-block-paragraph\">We support people of all ages, genders, and cultural backgrounds, with a particular focus on individuals experiencing increased vulnerability, including:<\/p>\n\n\n\n<ul class=\"wp-block-list is-style-imagedirect__list-underline\">\n<li>Young, adolescent, elderly (including living in residential care facilities)<\/li>\n\n\n\n<li>Farmers, and people affected with drought and bushfire<\/li>\n\n\n\n<li>People with financial, occupational or family\/ relationship issues<\/li>\n\n\n\n<li>People experiencing alcohol and other drug concerns<\/li>\n\n\n\n<li>People from CALD communities<\/li>\n\n\n\n<li>LGBTQIA+ individuals<\/li>\n<\/ul>\n<\/div>\n\n\n\n<h2 id=\"do-i-need-a-referral\" class=\"wp-block-heading\">Do I need a referral?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">You do not need a GP referral to access Head to Health Wellington.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">You can self\u2011refer by:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Calling our Sale office on (03) 5143 7984, or<\/li>\n\n\n\n<li>Calling the National Head to Health access line on 1800 595 212<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Our team will discuss your needs and help connect you with the most appropriate support.<\/p>\n\n\n\n<h2 id=\"outreach-service-co-locations\" class=\"wp-block-heading\">Outreach service &amp; Co-locations <\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Head to Health Wellington delivers the outreach support, to support individuals who may not otherwise be able to access mental health care.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Our outreach services include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Mental health support for residents living in residential aged care facilities<\/li>\n\n\n\n<li>Outreach support for older people living at home<\/li>\n\n\n\n<li>Co\u2011located outreach services in Maffra (Johnson Medical Centre) and Yarram<\/li>\n<\/ul>\n\n\n\n<h2 id=\"group-sessions-1\" class=\"wp-block-heading\">Group Sessions<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Our clinicians can provide group-based mental health sessions for vulnerable populations, covering a range of wellbeing and mental health topics.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">To enquire about group sessions or referrals, please contact our administration team.<\/p>\n\n\n\n<h2 id=\"privacy-and-confidentiality\" class=\"wp-block-heading\">Privacy and Confidentiality<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Head to Health Wellington complies with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs) in the collection, use, storage, and disclosure of personal and health information.
